Bridging Care Provider Preference

Bridging Care is a service that will give patients their post-biopsy results (negative & positive) typically within 48 hours of receiving the pathology results; provide customized resources for newly diagnosed patients; and schedule their first appointment for the next step in care.

This process will help connect patients with their next steps within their health providers’ system of care. The provider’s preference form will determine who the patients next appointment will be scheduled with please use the provider preference form below.
